Go to the "Moderator Applications" section, scroll down to find the format then copy & paste it into a new thread which you need to make using the "Create Thread" button in the sub-server you wish to apply for.

There are chances of you ./chest and ./pv's resetting if you are a donator. Possibly your bal and echest.
